This on-line course is designed for all local authority PASRR staff. It provides a basic overview of the PASRR Process by detailing the steps in the PASRR Process from nursing facility admission through the interdisciplinary meeting, including:

RE Screening, (PL1) & admission types;

PE (ID, DD & MI) factors resulting in positive status;

PE Role in sharing info re: community options;
-
Specialized services recommended by PE;
-
Assessments to support IDD Positive PE determination; and
-
NF’s IDT Meeting and initial identification of PCSP form.
Once you have completed the course, you must also complete the competency-based test before printing your certification of completion.
Please provide your supervisor and/or HR department at your agency with a copy of your certificate.

Welcome to the FY25 PASRR PL1 & PE (Instructor-led Virtual Presentation)

The purpose of this instructor-led presentation training is to explain to local authority staff how to complete the PASRR Level 1 (PL1) Screening Form and the PASRR Evaluation (PE) Form accurately.
To register for this course, you must have completed the course "An Overview of the PASRR Process FY 20XX".
Please note that this course requires the use of camera and audio during the virtual presentation. The PASRR Training unit currently uses Microsoft Teams as the platform when virtually presenting. Welcome to the FY 2025 PASRR Service Planning and Monitoring (Instructor-led Virtual Presentation for Habilitation Coordination) Course!

To register for this course, you must have completed the computer-based training "An Overview to the PASRR Process FY 20XX".
This course is for all local authority habilitation coordination staff. It is designed to instruct in service planning and monitoring required in the PASRR process and includes the following topics:
- Habilitation Coordination Refusals
- Community Living Options (CLO) including LIDDA Educational Opportunities
- Interdisciplinary Team (IDT)/Service Planning Team (SPT) requirement & representation
- Habilitation Assessment (HA)
- Form 1063, Individual Profile (IP) – Nursing Facility & Form 1057, Habilitation Service Plan (HSP) development
- PASRR Comprehensive Service Plan (PCSP) completion
- Ongoing follow-along with SPT involvement
- Special updates of HSP
- Quarterly review and revision of the HSP

Welcome to the FY2025 PASRR Transition and Diversion (Instructor-led Virtual Presentation for Service Coordination/ECC)

To register for this course, you must have completed the course "An Overview of the PASRR Process".
This instructor-led presentation, designed for local authority staff with PASRR transition or referrals responsibilities, instructs in the transition and diversion processes required for the PASRR process and includes the following topics:
- Duties related to transitional sections of the HSP
- Relocation Specialist Referral and MCO involvement
- Contract/Rule Review of duties related to Pre-Move requirements including slot requests (Diversion & Transition)
- Contract/Rule Review of duties related to Post-Move requirements. (Diversion & Transition)
- PSS entries in support of transitions
- Form 1050, Nursing Facility or Crisis Diversion Plan completion

Welcome to the "PASRR Transition and Diversion II: Instructor-led Virtual Presentation for Enhanced Community Coordination (ECC) FY 2025" Course!
The 2024 course is no longer available for enrollment, but you can still access your FY24 issued Certificate from your HHS Learning Portal Dashboard.
To register for this course, you must have completed the course "PASRR Transition and Division (Instructor-led Virtual Presentation for Service Coordination/ECC) FY 20XX" or an in-person training of the same course.
Note: This is an advanced course for ECC staff. Best practice is for the ECC staff to have at least 6 months providing services before registering for this course.
This instructor-led presentation is designed for all persons in the Health and Human Services (HHS) system who are involved in Nursing Facilities (NF), State Supported Living Centers (SSLC), and Intermediate Care Facilities for Individuals with Developmental Disabilities (ICF/IID) (Transitions and Diversions) responsibilities. It is an advanced course for Enhanced Community Coordination Coordinators with at least 6 months of experience providing services.

Welcome to the 'Community Options for People who have Mental Illness and an Intellectual or Developmental Disability: Hospital Social Worker Training' Course!
This optional training is designed to give social workers and other referring entities a brief overview of the PASRR process, and the tools, information and resources needed to successfully support a person with an intellectual (ID) or developmental disability (DD) and a mental illness (MI) transitioning from a hospital setting to a community setting. This course will also include a brief overview of roles for stakeholders, such as local authorities. By completing this course, social workers and hospital staff will be better prepared to support people in accessing appropriate community options and promoting independence, choice, and quality of life.
Once you have completed the course, you must also complete the competency-based test before printing your certification of completion.
After successfully completing the course and receiving a certificate of completion, please follow your agency's policy and protocols regarding who to provide your completed certificate.

The Preadmission and Screening Resident Review (PASRR) Level 1 (PL1): Back to Basics training reviews the PL1 form in a section-by-section manner. Participants will be asked to take notes on a blank PL1 form as they complete the training. At the conclusion of the training, participants will understand how to complete the PASRR Level 1 (PL1) screening form, and will be familiar with the following PL1 topics: Admissions, Transfers, Discharges, and Changes of Ownership (CHOW).
