Welcome to the Community First Choice in the HCS and TxHmL Programs FY 2024 Course!

The 2023 course is no longer available for enrollment, but you can still access your FY23 issued Certificate from your Dashboard.

This training is mandatory for all HCS and TxHmL service coordinators and includes a quiz with a minimum required score.

This training discusses Community First Choice as it applies to individuals receiving services through the HCS and TxHmL waiver programs. Topics in this training include CFC services, provider qualifications, eligible living environments, individual plan of care renewal, and other important CFC service coordination topics such as transportation, financial management services and monitoring.

Welcome to the Community First Choice in Medicaid Managed Care Organizations Course!

This training discusses Community First Choice as it applies to individuals receiving CFC services in Medicaid Managed Care Organizations. Topics in this training include CFC as it applies to individuals who are not enrolled in an IDD waiver program, CFC services, necessary assessments and documentation to facilitate the review and authorization for CFC services, and the LIDDA's responsibilities and processes for CFC services in an MCO. This training is required for LIDDA staff and service coordinators who facilitate non-wavier CFC services and processes and includes a quiz with a minimum required score.

Welcome to the Documentation of Outcomes in Person-Centered Plans FY 2024 Course!

The 2023 course is no longer available for enrollment, but you can still access your FY23 issued Certificate from your HHS Learning Portal Dashboard.

This is an optional course available to all persons in the HHS system who are involved in developing personal outcomes as part of a person-centered plan.

Topics include the value of discovery information to the development of personal outcomes and exploration of how the outcomes and pertinent information in the person-centered plan form the critical links to the development of short-termed objectives in the service provider’s implementation plan.

Start 'Documentation of Outcomes in Person-Centered Plans FY 2024' Course
 

Welcome to the HCS/TxHmL CFC PAS/HAB Assessment Form 8510 FY 2024 Course!

The 2023 course is no longer available for enrollment, but you can still access your FY23 issued certificate from your Dashboard.

This will be a required annual training over the Community First Community (CFC) Personal Assistance Services (PAS)/ Habilitation (HAB) assessment Form 8510 for the HCS/TxHmL service coordinators. The CFC training will cover the purpose of Form 8510, how to complete it, and when to prepare or update the CFC PAS/HAB Assessment along with a competency test. The IDD Services and the Utilization Review departments requests this training to assist service coordinators in how to complete the form correctly.

Welcome to Local Intellectual and Developmental Disability Authority (LIDDA) Coordination 101 FY 2024 course!

This is an optional course available to all persons in the Health and Human Services (HHS) system who are involved in service coordination.

This course explores coordination through five training modules: service planning and coordination, crisis prevention and intervention, monitoring, advocacy, and documentation. It is designed to give Coordinators the information, tools and resources needed to successfully serve people in a coordinator role.

Welcome to Outcomes: A Deeper Look at Documenting Outcomes in Person-Centered Plans

It is required that participants successfully complete the course “Documentation of Outcomes in Person-Centered Plans” prior to completing this course.

This optional course was developed for LIDDA staff who are involved in developing personal outcomes as part of a person-centered plan. This competency-based training focuses on making the connection between utilizing person-centered thinking skills, documenting discovery information, and developing personal outcomes that are truly driven by the person receiving services. The training emphasizes the importance of the discovery process and the relationship between outcomes and services.

Start Documenting Outcomes in Person-Centered Plans Part II Course

Welcome to the Permanency Planning FY 2024 course!

The 2023 course is no longer available for enrollment, but you can still access your FY23 issued Certificate from your HHS Learning Portal Dashboard.

This is an optional course available to all LIDDA staff who are involved in permanency planning.

This course explores the history of permanency planning for children with intellectual developmental disabilities, guidelines for the role of the planner, exploring alternatives in engaging families and young adults, understanding of what permanency planning is, and understanding the significance and importance of proper planning. The presentation piece of the course is the recorded Permanency Planning Training live webinar held on Tuesday, October 11, 2022.

It is designed to give those who create/update permanency plans the information, tools and resources needed to help families achieve life as an alternative to facility life.

Upon successful completion of this module, you will earn a certificate of completion.

Start Permanency Planning FY 2024 Course

Welcome to the Waiver Enrollment Training FY 2024

The 2023 course is no longer available for enrollment, but you can still access your FY23 issued Certificate from your Dashboard.

This training is designed to give HCS / TxHmL Enrollment Specialist the information, tools, and resources needed to successfully complete the HCS enrollment process, which must be completed for all people seeking enrollment into the Home and Community-based Services (HCS) and Texas Home Living (TxHmL) programs.

As required by Performance Contract, all enrollment staff at a local intellectual and developmental disability authority must complete training for waiver program enrollments before performing enrollment activities and at least annually thereafter for as long as the staff performs enrollment activities for the LIDDA. 

Upon successful completion of this module, you will earn a certificate of completion and (if applicable) continuing education credit.
